Subglottic Stenosis (SGS) Pasca Trauma Inhalasi

Latar Belakang: Laryngotracheal stenosis (LTS) terjadi pada 24-53% pasien pasca trauma inhalasi1. Insiden komplikasi pembedahan LTS adalah 33- 34% dan mortalitas 1,5-2%2. SGS sering cedera inhalasi intubasi1. Laporan Kasus: Pasien dengan luka bakar area wajah keempat ekstremitas, akibat ledakan tabung gas ruangan tertutup. Sembilan jam trauma, mengeluhkan kesulitan bernapas. diintubasi selama 2 hari 5 kali intubasi lainnya ETT cuff 6,5 mm untuk tindakan operasi. Tidak ada data tekanan pasien. Hari ke-38 perawatan di rumah sakit, mengeluh suaranya serak terkadang merasa sulit Hasil fiber optic laryngoscopy (FOL) menunjukan 30% penyempitan subglotis. didiagnosis stadium 1. tidak membutuhkan hanya diobservasi. Diskusi: Evaluasi FOL perlu dilakukan sejak awal inhalasi3. kami baru setelah muncul gejala SGS. Risiko meningkat sesuai keparahan inhalasi, reaksi inflamasi, durasi (lebih dari 10 hari), ukuran yang besar, berulang. Tekanan dapat mengakibatkan pembentukan skar subglotis1. direkomendasikan 20-30 cmH2O. diukur disesuaikan tiap 4-12 jam4. sebanyak 6 kali, tanpa pengukuran cuff. Stadium digunakan Cotton Meyer staging. 1 pembedahan5. Kesimpulan: Sekuel subglotis dicegah melakukan indikasi menggunakan kecil terlalu tinggi.
